Insurance
At Oral & Facial Surgery Center, LTD we make every effort to provide you with the finest care and the most convenient financial options. Dental Insurance is different from Medical Insurance. Many patients aren’t familiar with the specifics of their dental insurance benefits. Your dental benefits are based on the agreement your employer negotiated with your insurance carrier. Please review the insurance benefit booklet provided by your employer to better understand the benefits they make available as part of your insurance coverage. The patient payment portion covered for procedures varies depending on the coverage provided by your employer.
An estimate of the amount covered by your insurance company will be provided at the time of your treatment, based on the information they provide to us. The estimate is never a guarantee of benefits or the amount that will be paid. We will file all insurance claims as a courtesy to our patients. This does not however, transfer the responsibility of your financial obligation to the insurance company. If the amount paid by the insurance company is less than or greater than the estimate, then you will be billed the difference or issued a credit on the account in the event of an overpayment.

Who will be responsible for your account?
- A guarantor is the person held responsible for the patient’s financial account.
- The guarantor is ALWAYS the patient unless the patient is a MINOR (under age 18) or an incapacitated adult.
- The guarantor for a minor child is the parent or legal guardian that signs the CONSENT FOR SURGERY form and the PATIENT HEALTH HISTORY form.
- STUDENTS age 18 or older are their OWN guarantor.
- The GUARANTOR is NOT automatically the insurance subscriber or head of household.
- The “other parent” is NOT the guarantor. If the parent or legal guardian presenting the child brings a divorce decree stating the other parent is 100% financially responsible for the child’s medical bills, the guarantor will be changed to the parent designated in the divorce decree.
- Your spouse can be your guarantor ONLY if you are an incapacitated adult.
Appointment Policy
We try our best to stay on schedule to minimize your waiting. Because Dr. Jensen and Dr. Primavera provide many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
If an emergency arises and you are unable to keep your appointment, please provide at least 48 hours notice for any appointment that needs to be rescheduled.
